1. Complete the Affiliation Form in BLOCK CAPITALS and return to:

FBAS Treasurer, 15 Old Top Road. Hastings. East Sussex. TN35 5DJ.

2. Enclose a stamped, self-addressed envelope for your receipt, Delegate and Membership Cards.
3. The Affiliation Fee is due on the 1st of January each year.

Payment is by crossed cheque or Postal Order made payable to:
"Federation of British Aquatic Societies" or "F.B.A.S."

for £16-00 for UK Societies and £19-00 overseas Societies

NEVER send cash, uncrossed cheques or postal orders.

As an extra precaution you should write across the cheque or postal order between the crossings " A/C PAYEE "

4. Return your completed form as soon as possible.

DO NOT send a Trophy Application Form with the Affiliation Form.

Please note that no Trophy will be allocated to a Society that cannot quote the current year's Affiliation Number.

DO NOT SEND AFFILIATION FORMS
TO THE TROPHY AND BROOCH OFFICER OR
TROPHY APPLICATION FORMS TO THE TREASURER.

5. Societies not previously affiliated will be sent a free issue of selected Federation Books (Constitution & Show Rules, Year Book, Quiz Book 1 and National Show Fish Sizes). Only a number of National Show Fish Sizes are printed yearly and may not be in stock.
6. Should a Society require copies of any current Federation Books, these can be provided on a prepayment of at least 30% of normal cost. Please add 25% to the total cost of the items to cover post and packing.
7. Unless otherwise requested, by a Society on its official letterhead, all Federation information will be sent to the

Secretary listed on the Affiliation Form
Therefore any change of Secretary should be notified, to the Treasurer,
at once on a Society Letterhead.
Duplicate information cannot always be sent.

BEFORE POSTING PLEASE CHECK THE FOLLOWING:

ALL POSTCODES AND TELEPHONE NUMBERS INCLUDING STD CODES ARE LISTED.

IF E-MAIL ADRESSES ARE LISTED MAKE SURE IT IS CLEAR WHETHER UPPER OR LOWER CASE IS USED.

IS YOUR WRITING CLEAR AND READABLE?
UNLESS WE CAN READ IT WE CAN'T SEND YOU THE INFORMATION.

REMEMBER: A STAMPED SELF-ADDRESSED ENVELOPE MUST BE INCLUDED WITH THE CORRECT VALUE OF STAMPS ATTACHED.
